As a sub-specialist, I am painfully reminded on a daily basis that healthcare for many patients is fragmented. The historical source of fragmentation appears to be the free choice that patients had in selecting their few available physicians and hospitals, based on perceptions of quality, geographic proximity, and short-term relationships with insurance plans.
